Is it a straight bolt-on with minor modification or do you have to short/extend the frame as in a '89 ?

The frame does not have to be cut like the 89, but you do have to lay the metal in the fiberglass body, get the measurements and weld all the attachments.  The 78 Lincoln has the correct wheelbase for the 66.

What does lay the metal in the fiberglass body mean? So, you got the 2 door mark and stretched it is that right?
